    It's not really the forms that is vulnerable.
    If you can't input what you want there, you can always save the source to your HDD and modify it before doing what you need to do. The validation must be client side though for this to work since we don't have control over the server. Lastly, finding a vulnerable database is trial/error. Apart from low budget/self made site, the vast majority are now "patched" agains those attacks.
